Physical signs
Usually evident, persons have excessive sweating on the different areas described above.
Primary focal hyperhydrosis has to fulfill the criteria of excessive sweating for at least 6
months duration with no evident cause, and has at least two of the following:
• Impairs daily living
• Occurs on both sides of the body
• Frequently occurring, at least one episode a week
• First appeared at the age of less than 25
• Sweating stops during sleep
• Has relatives who has the same condition
Laboratories
If direct visualization of the affected area is needed the iodine starch test may be used.
This test requires spraying of the affected area with a mixture of 0.5-1 g of iodine crystals and
500 g of soluble starch. Areas that produce sweat will turn black.
• Complete blood count: to detect anemia and infection
• Erythrocyte sedimentation rate: to detect infection and inflammation
• Rheumatoid arthritis test: to test for rheumatoid arthritis
• Serum insulin assay and C-peptide: to identify insulinomas
• Culture of urine: to test for pyelonephritis
• Serum chemistry panel: to detect possible liver disease or kidney disease
• Drug screen: to identify if the symptoms are drug related
• Psycholigical/Psychiatric testing: to detect anxiety disorders or other psycholical/
psychiatric disease
